Assistant Care Manager/Care Manager

About the job

We are a hospital-based social work team supporting BCP residents following a hospital admission. We are looking for both full time and part time.

Your duties will include;

  • Multi-disciplinary working (Occupational Therapists, Physiotherapists and other medical professionals).
  • Risk assessing and managing positive risk.
  • Advocating – making sure people’s voices are heard, including carers.
  • Encouraging people to be independent, helping them grow their confidence and make the most of the support available to suit their individual needs.
  • Person centred discharge planning using a strengths-based approach.
  • Identifying what people and families are worried about or what they want to achieve; exploring what support they already have and what they may need.
  • Developing great knowledge of your local area and working in partnership with local providers and voluntary sector.
  • Providing creative solutions – out of the box thinking.
  • Making evidence-based decisions in a fast-paced environment.

 

We offer ongoing support, training and guidance to help you be the best you can be. But it will really help if you:

  • Hold or are working towards an NVQ level 3 in Health and Social Care or equivalent (equivalent must be health and social care related).
  • Have experience of working in a social care environment.
  • Are not afraid to do things differently to create support that is better for people.
  • Are passionate about advocating for others.
  • Are innovative, creative and solution focused in supporting people to be independent.
  • Are able to travel across the BCP area.

A bit about the Team:

We are based within University Hospitals Dorset (UHD) and work across health and social care services.

We are a highly motivated and supportive team, who work closely with our Health partners and community teams.

The team have a range of experience including Qualified Social Workers, newly qualified ASYE Social Workers, Apprentice/Trainee or Student Social Workers and a vast range of experienced unqualified Social Care staff.

This post will be predominantly based at Poole & Bournemouth Hospital sites but there will be a requirement to support the teams across all acute & community hospitals.
