The role:
As a Coordinator you will be responsible for the planning and coordination of reablement visits to maximise contact time and facilitate the support of new clients when required.
The Reablement service supports people following a life changing event or spell in hospital and the Coordinator role is central to the smooth running and delivery of support to people using our Reablement service.
Key responsibilities:
- Oversee the scheduling of staff team providing housing and community support, ensuring efficient and effective use of resources.
- Coordinate visits to minimise travel time and maximise visit capacity.
- Maintain clear and effective communication with clients, families, and healthcare professionals to ensure a coordinated approach to planned support visits.
- Support with reception and answering telephone calls.
- Ensure the smooth delivery of community and housing related support services with advance planning of rosters.
What we’re looking for:
- Diploma/NVQ Level 3 in Health and Social Care or equivalent
- Excellent communication and interpersonal skills
- Competent IT skills with experience using Microsoft systems including Word, Excel and Outlook
- Experience scheduling reablement community visits or a similar role
- Experience of using an electronic rostering programme
Full-time – 37 hours per week
