Continuing Health Care (CHC) - Bournemouth, Christchurch, and Poole (BCP) Council

Main Responsibilities
- Collaborate with NHS Dorset and other regions to complete Multi-disciplinary assessments for CHC eligibility.
- Support community Adult Social Care Teams in the CHC and Joint Funding process.
- Assess and support those with complex and diverse needs.
- Demonstrate alignment with BCP Council’s values and exceptional communication skills.

The Ideal Candidate
- Qualified Registered Social Worker.
- Passionate about providing excellent services in the field of Continuing Health Care
