Job Opportunity: Adult Social Care CHC Practitioner - Social Worker

Continuing Health Care (CHC) - Bournemouth, Christchurch, and Poole (BCP) Council

Role Purpose

Main Responsibilities

  • Collaborate with NHS Dorset and other regions to complete Multi-disciplinary assessments for CHC eligibility.
  • Support community Adult Social Care Teams in the CHC and Joint Funding process.
  • Assess and support those with complex and diverse needs.
  • Demonstrate alignment with BCP Council’s values and exceptional communication skills.
  • This is an exciting opportunity to join a highly motivated and supportive team.

The Ideal Candidate

  • You must be a Qualified Registered Social Worker.
  • Passionate about providing excellent services in the field of Continuing Health Care
