We are looking for a Coordinator to join our dedicated Reablement team.
As a Coordinator you will be planning and coordinating reablement visits to ensure clients receive, timely, effective support. You will ensuring that every person receives the care they need, when they need it.
Our Reablement team supports individuals following a life changing event or a hospital stay, helping them regain independence and confidence. Your coordination skills will be essential to keeping things running smoothly and ensuring every person receives the care they need, when they need it.
What you’ll do
- Oversee the scheduling of staff team providing community support, ensuring efficient and effective use of resources.
- Coordinate visits to minimise travel time and maximise visit capacity.
- Maintain clear and effective communication with clients, families, and healthcare professionals to ensure a coordinated approach to planned support visits.
- Support with reception and answering telephone calls.
- Ensure the smooth delivery of community and housing related support services with advance planning of rosters.
What you need
- Diploma/NVQ Level 3 in Health and Social Care or equivalent
- Excellent communication and interpersonal skills
- Competent IT skills with experience using Microsoft systems including Word, Excel and Outlook
- Experience scheduling reablement community visits or a similar role
- Experience of using an electronic rostering programme
Full-time – 37 hours per week
